Now Accepting New Patients

The Port Hope Community Health Cente serves all residents without a local family physician.  Special Consideration is given to the elderly with chronic conditions, youth at risk, those struggling with mental health issues or additions and those living on a fixed or limited income.

Community outreach programs and workshops including diabetes education, support, counselling, nutrition education, health education, and lifestyle programs are open to everyone and not limited to those registered as clients of the centre.

The Centre is not a walk-in clinic.

Once you drop off or mail in your registration form you can expect to receive a call from one of our medical support staff to set up your initial consultation. Your health needs are important to us and we look forward to meeting your in the near future.

As a new health care  provider in the community we have received a high number of registrations over the last several months. Unfortunately, this has caused some delay in getting new patients in our Centre. You will hear from us. Our goal is to provide you with the best medical care and range of supports to promote illness prevention and good health.
