General Information

We offer services primarily from our location at 99 Toronto Road. We are accepting applications for primary health care for people without a family physician living within a 50 KM radius of the CHC.

Special consideration will be given to our target populations who do not have a family physician. These include:

• Seniors with more than one chronic disease,
• Youth at risk between the ages of 12 to 21
• Those struggling with mental health and/addictions
• Those receiving social assistance

Our workshops and programs are open to the general public, registration is required.
