What is the Port Hope Community Health Centre?
The Port Hope Community Health Centre is a non-profit, community governed healthcare and support service centre. It serves residents within a 50 kilometer radius of the Centre who do not have a family physician or who need its support and wellness services.
Who runs the Port Hope Community Health Centre?
The Centre is governed by a volunteer Board of Directors of individuals from the community. Currently, 13 directors are responsible for the mission and strategic direction of the Centre.
Does the Centre have a walk in clinic?
The Centre does not provide emergency care, nor is it a walk-in clinic. Although same day visits may be available, clients must make an appointment. Primary care and support services are available to those registered with the Community Health Centre. If you require emergency care please go to the hospital.
What if I need to see a health professional on short notice?
Every effort will be made to leave appointment time slots open and available for clients needing urgent care. Clients must call the Centre to obtain urgent care appointments.
What are the types of health professionals and other staff that provide service?
The Centre has family physicians, nurse practitioners and a healthcare support team providing services such as food care, mental health counseling, diabetes education, health promotion, caregiver support dietician and community outreach programs.
Do I have to pay to receive services?
There are no fees for any services which are covered by OHIP.
Are you open evenings and weekends?
The Centre is open Monday, Tuesday, Wednesday and Friday from 8:30am to 5:00pm, Thursday 8:30 to 7:00pm
What if I miss an appointment?
Clients are asked to make every effort to keep appointments and to be on time. If you cannot make an appointment we request 24 hours notice. If you miss three consecutive appointments your registration may be voided.
Can I go to another general practitioner while I am a client of the Health Centre?
When you become a client of the Community Health Centre you are enrolled with the physicians at the Centre. If you choose to see another general practitioner the physician with whom you are enrolled can remove you from the roster.
