Client Rights & Responsibilities
Every Client has the right to:
- be treated in a courteous and respectful manner
- be treated in a manner that ensures privacy, dignity and promotes autonomy
- be treated in a manner that is sensitive and responsive to your needs and preference including ethno-social, spiritual and cultural
- to receive information about services that is clear so that you can make informed choices and decisions
- be informed as to who will be providing your services
- participate in the development and evaluation of your service plan
- give or refuse consent to services
- feel free to raise concerns or request changes to any aspect of the services provided
- be provided a response to all concerns expressed
- understand the complaints procedure
- be provided information on legislation, PHCHC policies as it relates to client service
- have personal records kept confidential in accordance with legislation
Every Client has the responsibility to:
- treat other clients and staff with dignity, respect and courtesy
- respect the privacy of other clients
- provide all required information and sign necessary releases
- inform the receptionist of any changes in your personal information such as telephone number, address etc
- provide complete and honest information about your health history and status
- follow health advice and medical instructions
- report any significant changes in symptoms or failure to improve
- seek clarification on any information provided that is not understood
- actively participate in health education and seek information concerning your health
- express any concerns or issues with your health provider
- be aware of the complaints procedure
- make every effort to keep appointments and be on time
- inform the PHCHC when you are unable to attend appointments or programs.
