Application Process
The Port Hope Community Health Center continues to accept new clients for primary health care services. Primary health care services respond to clients’ needs from infancy to old age.
You do not have to be a registered client of the centre to receive service from the Diabetes Nurse Educator or to participate in any of the workshops or education series offered at the Centre.
You can come into the Centre and complete an application or download the form and drop it off at the Centre during office hours.
