Our Commitment
The CHC is committed to respecting your privacy and protecting your personal health information.
The Ontario Personal Health Information Protection Act (2004) is the foundation of the CHC privacy policies.
The CHC collects your personal health information, including your name, address, gender, age and health card number (HIN) to:
• Determine you eligibility for service
• Plan your health care
• Record care and service provided
• Accurately communicate between other health care providers involved with your care
• Assess and review the quality of the care and service we provide
• Review care and service for the purpose of organization wide quality improvement activities so that we can meet the needs of our clients and the communities we serve
• To submit information required by funding agencies e.g. The Ministry of Health and Long Term Care
Sharing your personal health information
The CHC will not share your personal health information without your permission unless required by law.
